The purpose of the Training, Education, Access, and Management for a Life that is Sugar Free (TEAM Sugar Free) program is to improve the health of people with diabetes in the Mississippi Delta and North Mississippi. The TEAM Sugar Free program is a collaboration between regional and community partners working together to combat diabetes in our target area.

Pharmacy Intervention

The University of Mississippi Medical Center Department of Pharmacy currently collaborates with the Delta Health Alliance for a Medication Management program supported by Electronic Health Records (EHR's) in Coahoma, Panola, and Yazoo counties. Funds from the BLUES Beacon Community project will serve to expand this initiative to include new EHR locations and network with the Health Information Exchange system expansion through telephonic patient care management.

Health Information Exchange

A Health Information Exchange (HIE) is an electronic network that facilitates the movement of healthcare information electronically across organizations within a region, community, or hospital system. It is the goal of Beacon to connect all providers with electronic health records to a centralized, statewide HIE, which DHA will coordinate for all providers who serve Delta patients.
